Medicaid Waiver Program
Qualifications: The Director of Supported Living Services will have a Bachelor’s degree in a Human Service field with a minimum of two years experience in a supervisory capacity and two years of direct experience in support services to people with developmental disabilities.
The Director will have a working knowledge of all aspects of individualized supported living, day services community resources, and the certification / accreditation process. The Director will have a working knowledge of the service delivery system including trends in community inclusion. The Director will demonstrate skills in personnel management, organization, service coordination, and advocacy. The Director will possess good interpersonal skills and be familiar with basic computer applications, budgeting, and program development.
Function: The Director of Supported Living Services is responsible for the development and management of the Willows Way ISL / Medicaid Waiver services. The Director will monitor the quality and consistency of the ISL / Medicaid Waiver services to insure outcomes for people are successful and customer satisfaction is maintained.
The Director of ISL Services will be responsible for assuring that all necessary on and off-site support, supervision and administrative action is provided to insure that all staff on the service delivery team are adequately trained and supported to carry out their responsibilities.
The Director’s position emphasizes building a community not only with and among customers, but also with parents, staff, board members, benefactors and friends of Willows Way.
Accountable: Assistant Executive Director
Supervises: Program Managers/QMRPs
Working Conditions: The Director will assure that 24 hour support, including on-call or emergency contact systems are consistently provided through the ISL program to insure that all customers’ health and safety and support needs are assured.
Responsibilities: To lead and direct the service delivery team in supporting each individual so that he/she will feel a sense of self-determination, security and satisfaction in their lifestyle, have opportunity to have meaningful relationships with others in their community, have his/her rights and dignity respected at all times and have a healthful, safe home and lifestyle, the Director of Supported Living Services will:
• insure that adequate, competent staff are available to provide the necessary supports and supervision to each individual served. This will include constructing and reviewing staffing schedules with program supervisors, identifying staffing needs to the Human Resource Department, interviewing, coaching, orienting, training, and providing necessary performance discipline to all staff supervised.
• assure provision of 24-hour response systems to customers, employees and families.
• conduct weekly review meetings with supervisory/QMRP to assure delivery of services and maintenance of quality supports
• insure that weekly support team meetings are conducted among advocates and program supervisors and that current administrative and operational information is clearly and accurately communicated
• Provide oversight of new employee training/documentation and annual employee evaluations to include scheduling of review and all data flow documentation
• Attend customer PCP/team meetings as necessary
• Review all Quality Assurance / enhancement reports, assuring follow-up on all action plans
• Ensure that the service delivery team actions comply with all DMH Certification Principles and all Outcome Standards of The Council by reviewing all relevant documentation, observation of staff performance and training/modeling of the principles/standards. Will provide necessary assistance in all state programmatic surveys and federal audits
• Coordinate referrals for services and transition planning to include: completion of application process, case record development, case coordination, person centered planning, housing, roommates, start-up resources, and the orientation of new customers
• Insure that all staffing plans and financial support information for each customer served is kept up to date to meet each customer’s needs and that this information is accurately communicated to the Willows Way business office for individual budget planning
• Conduct investigations according to Willows Way policy into any alleged abuse or neglect incident as assigned by the Assistant Executive Director
• Submit informational reports as requested to the Assistant Executive Director in an accurate and timely manner
• Be responsible for completing, or assuring the accurate completion of all payroll documentation to include, time cards, data flow changes, ADP separation documents, PLT requests, etc.
• Develop and implement systems and procedures that enable service delivery team members to complete their responsibilities effectively and efficiently
• With the support of Willows Way Training Coordinator develop necessary training and resources for staff to insure that customers receive the highest quality of services, and will monitor all employees training to insure the completion of all required trainings
• Participate in all necessary management meetings to communicate information, facilitate planning and decision-making regarding ISL program and agency operations
• In the absence of the Assistant Executive Director, will act as the designee
• Attend agency staff meetings, committees, participate in agency sponsored inservice training, and independently enhance personal/professional development formally and informally.
• In conjunction with Willows Way HR and WW Training Coordinator assure that all ISL employee training and personnel records are kept complete and up to date.
• Implement Willows Way policy and procedure, OSHA standards and to conduct all operations as safely and efficiently as possible
• Adhere to and promote the ethical behavior guidelines for Willows Way employees and assure that all staff supervised adhere to these guidelines
